PARENTS CORNER

Eligibility

Lily Pad Learning Center accepts children between the ages of 6 weeks to 12 years. Priority is given to siblings of children currently and previously enrolled with good or past standing with the daycare (behavior of children and on-time tuition payments, etc.).

Enrollment Procedures-Contract

Setting up an appointment with our director, who will then connect you with the teacher responsible for your child/children on a daily basis, will be the first step, along with a brief tour and orientation. Bringing your child/children along for one of your visits will allow them to see our facility, meet their teacher, and for all of our staff members to get to know a new face(s)!

Parents who then choose to enroll their children at Lily Pad Learning Center, must sign a contract which will indicate attendance schedule, services provided by LPLC, along with the associated tuition and fees and who will be responsible for all billing and correspondence associated with tuition payments.

A packet with a checklist of forms to be completed upon enrolling will need to be filled out by the parent/guardian and returned in order to be placed in the child(ren)’s file upon day 1 of attendance. Items in this packet include medical information, release signatures for medical care, media, etc., a copy of a certified birth certificate (we can make this copy for you if needed-we DO NOT need an official copy as a fee is required to obtain them at each courthouse), and several additional forms (forms are kept confidential at all times).

All forms must be keep updated at all times. If an address, phone number, etc. changes, parents/guardians are required to update their paperwork immediately. In addition, payment for contracted days will be required regardless of actual attendance. Information concerning the admission, progress, health, or discharge of a child shall be confidential and limited to staff, parents, and representatives of the state unless the parent grants specific written permission for disclosure to other parties.

*The enrollment forms and fee agreements are not meant to serve as contracts guaranteeing service for any duration. Lily Pad Learning Center reserves the right to dismiss any parent or child at any time with or without cause.

Waiting List

A child/children’s name(s) will be placed on our waiting list if space is not available at that time. Payment will need to be made in the amount of one week’s tuition along with the non-refundable deposit. Notification to parents/guardians will be made as soon as a spot opens up.

The option will be given, when space comes available, to start payment immediately (even if you’re not ready/prepared to accept the space at that time). The spot will be given to the next name on our list if you choose not to hold the space for your child. The week's worth of payment, in this situation, will be refunded but the deposit will not OR a family may keep their child(ren) on the waiting list until another spot would open up.

State Aid

Parents that request to utilize Community Connection Point (CCP), Illinois’s state aid assistance service will need to request an application from them by calling: 217-525-2805 or by going on their website at: ccpoint.org. Fill it out, bring it to a director at Lily Pad for them to fill out our portion of the application (IL only). Parents will then be responsible for mailing the application, along with the additional requested information needed, which is stated on the application.

Missouri parents will need to contact DSS by calling: 855-373-4636 or apply on the website. 

Parents will be responsible for paying the co-pay amount (IL) and the sliding and co-pay fee (MO) that the state assigns them on the 1st of every month for Illinois families and every week for MO families. Parents will also be asked to sign a release form that will go in their child(ren)’s folder stating that they would become responsible for daily payments should the state of Illinois or Missouri ever stop making payments to daycare providers or their child(ren)’s spot(s) at Lily Pad Learning Center will be terminated.

Parents (MO) are responsible for signing their child(ren) in and out each day of attendance or will be required to pay a $1 per day fee. Signing in and out IS REQUIRED by the state in order to receive this service.
